A total of 22 teams contest the division, including 17 sides from last season, four clubs promoted from Division Ones plus one club who had been transferred from the Northern Premier League:
- Didcot Town, promoted as play-off winners in Division One South & West
- Hednesford Town, transferred from Northern Premier League
- Leamington, promoted as champions of Division One Midlands
- Nuneaton Town, promoted as play-off winners in Division One Midlands
- Truro City, promoted as champions of Division One South & West
Farnborough won the Southern League Premier and were promoted to Conference South, while play-off winners Nuneaton Town achieved the second promotion in two seasons after club reorganization to return in Conference.
Clevedon Town and Rugby Town were the only clubs relegated this season, while Merthyr Tydfil were liquidated and reborn under the name Merthyr Town three divisions below Southern League Premier Division. Thus, two clubs, Tiverton Town and Hemel Hempstead Town were reprieved from relegation.

Merthyr Tydfil were later expelled from the league after failing to meet a FA deadline to give assurances that they could trade effectively during 2010–11.[1]

- Beaconsfield SYCOB, transferred from Division One South & West
- Biggleswade Town, promoted as champions of Spartan South Midlands League

- Hitchin Town, relegated from Premier Division
- Slough Town, transferred from Division One South & West
Bury Town won the division in its second season in Southern League and returned to Isthmian League to take a place in Premier Division, while play-off winners Chesham United went back to Southern League Premier Division after three seasons of absence. Aylesbury United finished bottom of the table and were relegated to the lower leagues along with Rothwell United resigned from the league at the end of the season. Thus, Barton Rovers finished second bottom were reprieved from relegation.
